INSTRUCTIONS ON HOW TO REGISTER FOR A COVID-19 VACCINE FOR INDIVIDUALS WITHOUT A PESEL NUMBER

All individuals who have applied for vaccination at the University Vaccination Point and have been verified as students, doctoral students or employees of the University of Warsaw and do not have a PESEL number will be issued a qualifying referral for COVID-19 vaccine.

The referral will be issued no later than on the day of vaccination at the University Vaccination Point.


REGISTRATION FOR COVID-19 VACCINATION APPOINTMENTS

All individuals who applied for vaccination on the form provided by the University of Warsaw (previously available online from May 6-12) will receive an email and/or text message* 24 hours prior to the start of the online registration period.

Registration will only be available to individuals who have previously reported their intention to be vaccinated at the University Vaccination Point and will be possible in 3 ways:

1. Via the online registration system on a dedicated website
2. Through the automatic telephone registration system
3. By calling and speaking directly with a consultant

Upon successful registration for a COVID-19 vaccination appointment, all registered individuals will receive confirmation of the assigned vaccination date and time via text message and an email.

Individual registration for appointments will be possible after confirmation of the vaccine delivery date.We anticipate that the registration will start no later than May 31, 2021.

As of May 19, 2021, we have received information that administering the first dose of the vaccine should take place no later than June 17-26, 2021. The deadline for administering the second dose is approximately 35 days after taking the first dose.
