QUALIFICATION QUESTIONNAIRE

INSTRUCTIONS ON HOW TO REGISTER FOR A COVID-19 VACCINE FOR INDIVIDUALS WITHOUT A PESEL NUMBER

All individuals who have applied for vaccination at the University Vaccination Point and have been verified as students, doctoral students or employees of the University of Warsaw and do not have a PESEL number will be issued a qualifying referral for COVID-19 vaccine.

The referral will be issued no later than on the day of vaccination at the University Vaccination Point.


REGISTRATION FOR COVID-19 VACCINATION APPOINTMENTS

All individuals who applied for vaccination on the form provided by the University of Warsaw (previously available online from May 6-12) and were verified will receive a confirmation email and/or text message on the 31st of May 2021.

ON THE 2ND OF JUNE AROUND 6:00 PM WE WILL SEND ANOTHER MESSAGE WITH A DEDICATED LINK WHICH WILL ENABLE CHOICE OF DATE AND HOUR OF THE VACCINATION APPOINTMENT.

We have confirmation of Pfizer vaccine delivery:

First dose will be provided between 10th and 14th of June 2021
Second dose will be provided between 14th and 19th of July 2021



Choice of date and hour of vaccination will be possible:

1. Via the online registration system on a dedicated website – personal link will be sent to mobile phone and/or email on the 1st and 2nd of June

2. By calling and speaking directly with a consultant (after first stage of online choice)

Please be conscious of the high demand for COVID-19 immunization and submit your cancellation request via email at uw@centermed.pl Include the following information in your cancellation request:
